How to file an Accident Claim

When an injury occurs, the parent or guardian must complete the Youth Soccer Accident Proof of Loss claim form, which is obtained from the secure section of Florida Youth Soccer Association's website through your club's Agent of Record, President or Registrar. The form must be signed by the Coach, the Agent of Record from your club, and your parent or legal guardian.
