If you are a member of Massachusetts Youth Soccer and you plan to sponsor or host a camp/clinic, you can purchase low cost insurance to cover the participants, the event itself and you or the sponsoring organization.

For this reason, Bollinger offers the option to purchase insurance for camps/clinics for association members.

Program Limits:


Accident Insurance
  • $ 25,000 Medical/Dental Maximum
  • $ 1,000 Limit for Physical Therapy/Chiropractic Treatment
  • $ 1,000 Accidental Death & Dismemberment Benefit
  • $ 250 Deductible - per injury
  • 52 Week Benefit Period
  • Full Excess Coverage

Liability Insurance
  • $ 5,000,000 Aggregate Limit
  • $ 1,000,000 Per Occurrence Limit (incl. Participant Liability)
  • $ 1,000,000 Products/Completed Operations Aggregate
  • $ 1,000,000 Personal Injury/Advertising Injury Limit
  • $ 100,000 Fire Legal Liability Limit
  • $ 5,000 Medical Payments (for non-participants only)
  • $ 0 Deductible
  • Sexual Abuse & Molestation is excluded

How to Apply:

  1. Complete the Premium Calculation forms below.

  2. Include a roster of all participants. If you have your own spreadsheet or roster form, you may include that with your application. Or, if you would like to use the Massachusetts Camp/Clinic Roster form, Download the form .

  3. Send these three items along with your premium check to Bollinger, ATTN: Brenda Lodato, PO Box 390, Short Hills, NJ 07078.

  4. We must receive your premium prior to the start date of your event in order for coverage to be bound.
